Complete and submit an Application for Graduate Admission.
Provide official transcripts from a regionally accredited Baccalaureate degree granting institution.
Provide official transcripts from all regionally accredited institutions where post-baccalaureate coursework was completed or attempted (where you were granted a Master’s degree). Candidate must have a Master of Science in Nursing degree from a Commision on Collegiate Nursing Education (CCNE) or a National League of Nursing Accreditation Commission (NLNAC) accredited program with Advanced Practice Specialty [Nurse Practitioner (NP) Clinical Nurse Specialist (CNS), Nurse Midwife (CNM) or Nurse Anesthesia (CNA)].View Graduate Admission Options.
International applicants or domestic applicants with transcripts from international institutions must also submit foreign transcripts. PLEASE DO NOT FAX TRANSCRIPTS.

With the exception of transcripts and test scores, you may also fax additional required documents directly to the Office of Admission at (866) 659-1143 or e-mail the documents as attachments to email@example.com.
Have certification as an advanced practice nurse (or eligible to take National exam if CNS).
Submit a current and detailed resume or curriculum vita.
Complete the Self Evaluation and Goal Statement.
Submit three Reference Forms from any of the following:
your most recent employer/supervisor
A professor who can assess your clinical performance and judgement in the area in which you wish to study (only if you've graduated from a nursing program within the last five years)
A professional person (not a relative) such as a Professional Nurse or Advanced Practice Nursing colleague who is well acquainted with your professional practice and academic achievement.
Submit a copy of an unencumbered RN license from the state you pland to do your clinical hours.
Submit a copy of active Advance Practice RN License in state where you plan to do your clinical hours (not required of CNS if not licensed by their state).
Interview (by phone or in person) as needed.
Additional laboratory tests/and or immunizations, or documentation may be required for clinical residency hours by certain health care agencies if you are not an employee including an annual flu vaccine.
