Students desiring and requesting academic support services for disabilities are required to submit appropriate documentation to verify eligibility under Section 504 of the Rehabilitation Act of 1973 and the Americans with Disabilities Act of 1990. All assessment reports, medical information and other related documentation will be kept confidential at The Office of Disability Services in compliance with the Family Educational Rights and Privacy Act (FERPA), as well as in accordance with the professional and ethical standards of the Association on Higher Education and Disability (AHEAD). The following guidelines are provided to insure that documentation is sufficient and appropriate to verify eligibility for disability services.
Documentation on file may consist of the following:
- Statement of the student's present level of functioning and how his or her life functioning is impaired
- Veterans Affairs documentation
- A complete educational, psychological, developmental, and/or medical report indicating disabilities
- Describe the specific accommodations requested and detailed explanation of why each accommodation is recommended.
