If you have completed the FAFSA and you have received a Student Aid Report that indicates you have been selected for verification, you must complete a Verification Worksheet. Please select the type of Verification Worksheet that applies to you and attach a signed copy of your federal tax transcript, following the instructions on the worksheet. If you did not file taxes, please include your W-2 forms.
After you open the appropriate verification worksheet, you will need to print it out, complete all sections and sign it. Please mail your paperwork to:

Fill out the Independent form if you meet at least one of the following conditions:
Born before January 1, 1990 Veteran of the U.S. Armed Forces or currently on active duty Enrolled in a masters or doctorate program at the beginning of 2013-2014 Provide more than 50% of a dependent child’s support Provide more than 50% support of a dependent who lives with you Married Orphan Ward of the court (now or was through age 18) or were in foster care after age 13 Emancipated minor In a legal guardianship as determined by a court in your state of legal residence Unaccompanied youth who is homeless
Fill out the Dependent form if you do not meet any of the conditions above.
